Position Summary: Support the well-being and social inclusion of participants through assessment, work planning, and network coordination, applying social instruments and intervention strategies. Fundación Hogar de Cristo, under Law No. 21.015 promoting the labor inclusion of persons with disabilities, seeks to fill the position of Social Worker to work in Talca. **Objective**: Support the well-being and social inclusion of participants within the scope of practice, through appropriate assessment and a needs- and aspiration-centered work plan (participatory). This plan should facilitate coordination and management of necessary networks to meet participants’ needs—including connections with significant others, access to goods, services, and civil and social rights. Likewise, it supports and guides the appropriate application of social instruments and implements individual and group intervention strategies, in accordance with the guidelines set forth in the respective Technical Model. **Functions**: * Plan and conduct meetings with participants to foster relational bonds. * Conduct social assessments of participants to address crisis situations, provide containment, and refer individuals requiring more complex or specialized care to appropriate services. * Coordinate social program services according to participants’ needs, adhering to defined standards. * Conduct social assessments using social work techniques such as interviews, fieldwork, or others. * Develop individual, group, and community intervention plans per the Technical Model’s guidelines. * Manage and coordinate with public, private, and civil society institutions forming part of the network serving vulnerable individuals, as well as other networks enabling joint action toward social inclusion. * Guide, advise, and support frontline staff in their interventions with participants. * Conduct periodic field visits to identify the contexts in which program participants live and operate. * Perform night, weekend, and/or holiday shifts as required and upon prior coordination with supervision. * Efficiently manage material and informational resources assigned to achieve goals established by the Foundation’s Management and strategic plans of Fundación de Beneficiencia Hogar de Cristo and its subsidiaries. * Comply with Hogar de Cristo’s ethical and compliance frameworks and act in accordance with the current Crime Prevention Model developed under Law No. 20.393, safeguarding institutional responsibility. **Requirements**: * Social worker (with bachelor’s degree). * Knowledge of the public service network. * Experience in network-based collaboration. * Knowledge of poverty, vulnerability, and exclusion issues. * Minimum one year of experience working with individuals in conditions of extreme poverty and vulnerability. **Schedule**: Monday–Thursday, 08:30–18:00 hrs; Friday until 17:00 hrs. **Location**: Talca **Contract Type**: Fixed-term contract with potential extension to indefinite term.
