Your mission will be to conduct methodical social interventions and assessments aimed at promoting timely access to healthcare benefits and programs, managing support networks, and performing continuous technical follow-up to contribute to comprehensive and person-centered care. Position Purpose: To conduct methodical social interventions and assessments to guide case management and coordinate support networks ensuring access to healthcare programs and close, effective care. Key Responsibilities: Social Care and Intervention: Conduct interviews, provide guidance, and prepare social diagnoses and intervention plans in accordance with technical guidelines.
