This program is implemented through three components: Care Plan, Home-Based Care Service, and Specialized Support and Care Services. Additionally, the program includes a cross-cutting strategic action line of Community Management. The program's objective is to "reduce the biopsychosocial vulnerability of the caregiving dyad, whereby the functionally dependent person improves or maintains their level of functionality and their primary caregiver reduces or maintains their burden." Its specific objectives are: To develop an intervention guide for accessing services and benefits that reduce the biopsychosocial vulnerability of the caregiving dyad. The application requirements are as follows: Curriculum Vitae, photocopy of national ID card, certificate of special-purpose background check, certificate of disqualification for working with minors, professional degree certificate issued by a university or state-recognized professional institute in Chile, certificates of courses or specializations related to the subject matter, residency certificate in the commune of San Clemente.
