Position Objective: Identify, evaluate, and prevent various social and/or family-related factors, aiming to humanely provide tools that promote social reintegration and user adherence and retention in dialysis treatment, aligned with department objectives and DaVita's corporate guidelines. Main Responsibilities: * Develop strategies for social support and integration for DaVita kidney failure patients * Promote and participate in group educational activities according to priorities established by the psychosocial department, applying group and community education methodologies and techniques * Prepare informational materials about relevant social benefits available to users and their primary support network * Coordinate externally with health institutions and internally through multidisciplinary approaches regarding users' social issues * Prepare and produce social reports as required, which may include: disability certification, management of nutritional supplements, case-specific social benefits, transportation assistance as internal benefits * Conduct evaluation and delivery of social benefits through the healthcare network * Accompany users in processes of social, educational, and labor integration * Produce monthly visit and management reports * Define and analyze social risk factors affecting health conditions and develop intervention plans with an interdisciplinary and cross-cutting perspective * Provide attention and follow-up for social cases referred from programs or identified through spontaneous user requests * Maintain permanent coordination with public and private intersectoral institutions * Participate in working groups convened by various public and private institutions deemed relevant by the psychosocial department * Work collaboratively in psychosocial pairs for mental health interventions, as well as with teams from other areas, in support of strategic objectives and the company’s mission: The Patients.
